CareSource has finalized its affiliation with Community Care, a Brookfield, Wis.-based managed care organization serving older adults and adults with disabilities.
Community Care serves Wisconsin adults 55 and older, as well as adults 18 and older with physical, intellectual or developmental disabilities, through three of the state’s long-term care programs. The organization also operates adult day and health centers in Milwaukee, Racine and Waukesha. Community Care and its employees will remain in Wisconsin.
The deal marks CareSource’s second affiliation with a Wisconsin company, completing a deal with Brookfield-based Common Ground Healthcare Cooperative last year. A planned affiliation with Fond du Lac-based Lakeland Care was cancelled in September 2025.
Last year, CareSource also closed on its acquisition of Boston-based Commonwealth Care Alliance and New York City-based ElderServe Health. The insurer has more than 2 million members nationally.
