UnitedHealth Group in the headlines: 10 updates

From UnitedHealthcare announcing plans to cut prior authorizations by 30%, to Optum Rx planning to delink drug prices from fees, here are 10 updates on UnitedHealth Group and its subsidiaries that Becker’s has reported since May 1:

1. UnitedHealth Group is monitoring some Optum staff’s AI use, sources familiar with the matter told Bloomberg

2. Tim Spilker, who spent 18 years across Optum and UnitedHealthcare, has stepped down. Mr. Spilker said his most recent role, CEO of Optum Whole Health Solutions, focused on behavioral and population health solutions, but he previously led Medicaid and government programs. Before his role at Optum, Mr. Spilker was CEO of UnitedHealthcare Community & State. 

3. UnitedHealth Group is one of five payers named to Time’s “World’s Most Impactful Companies of 2026″ list. 

4. Optum Rx, UnitedHealth’s pharmacy benefit manager, said it will roll out a fee-based structure to customers in the coming years. 

5. UnitedHealth topped payers’ first-quarter profits, with net earnings of $6.3 billion in the three months ended March 31. 

6. UnitedHealthcare has begun paying some commercial claims instantly, bypassing the standard three to five day window associated with traditional ACH transactions.

7. UnitedHealth Group CEO Stephen Hemsley had the highest CEO-to-median employee pay ratio in 2025. The company noted, however, that a significant portion of Mr. Hemsley’s compensation was due to his one-time stock option grant at the time of his appointment and the company anticipates the CEO to median employee pay ratio “will decrease significantly next year.”

8. UnitedHealthcare is paring down prior authorizations for 30% of services with these requirements. The cuts will take place by the end of 2026. UnitedHealthcare will roll back authorization requirements for some outpatient surgeries, diagnostic tests like echocardiograms, outpatient therapies and chiropractic care. 

9. Pittsburgh-based Highmark Health named former UnitedHealth executive Heather Cianfrocco as COO. Prior to her new role, Ms. Cianfrocco served as executive vice president of governance, compliance and information security for UnitedHealth Group. She also served in multiple roles at Optum, including CEO, president, and CEO of Optum Rx. 

10. UnitedHealth Group plans to acquire Alegeus Technologies, a health benefits administration platform, the company disclosed in its first-quarter earnings.
