Humana and I-SNP carrier Longevity Health have extended their partnership through 2030.
First launched in 2023, the collaboration expands special needs health plan offerings for Medicare-eligible individuals living in skilled nursing and senior living facilities. The collaboration currently serves members in seven states, with plans for expansion to new markets and customer segments underway, according to a June 3 news release.
Longevity and Humana partner with skilled nursing facilities via a value-based model that rewards facilities for improving quality of care and member experience. Longevity provides services that address the medical, social and emotional needs of members, typically individuals with multiple complex conditions.
