Orange, Calif.-based Medicare Advantage company Alignment Healthcare's program for chronically-ill seniors has grown to serve more than 4,000 high-risk members since launching in 2017, the company said May 26.
The "Care Anywhere" program has helped reduce hospitalizations, emergency room visits and skilled nursing services among members when compared to Medicare fee-for-service performance, the company said, citing CMS data.
The program has also received a Net Promoter Score of 76, nearly double the average industry score of 30 to 40.
"This high satisfaction is a direct result of Alignment's Care Anywhere model, which delivers coordinated and preventive care proven to optimize clinical outcomes while driving down care costs," a news release said.
The average Care Anywhere patient is 77 years old and has up to six chronic health conditions.
