The collaboration, called the “Neighborhood Health Compass,” will use the technique of analyzing data from both clinical and non-clinical sources to identify frequent healthcare users and improve their care.

The National Center for Complex Health and Social Needs is an initiative of the Camden Coalition of Healthcare Providers. The coalition’s vision is to make Camden, N.J., the first U.S. city to “bend the cost curve while improving quality,” in healthcare, according to the organization’s website.
