1. Centene Corp.: $216.9 million, or about $420 per member
2. Norfolk, Va.-based Sentara Healthcare’s Optima Health: $98.9 million, or about $1,850 per member
3. Chicago-based Health Care Service Corp.: $78.5 million, or about $180 per member
4. Cigna: $55.9 million, or about $320 per member
5. Highmark: $50.8 million, or about $1,250 per member
