HealthPartners to launch hybrid, prevention-focused care plans

HealthPartners, an integrated payer-provider organization, is launching a new hybrid preventive care offering designed around health screenings, immunizations and other routine care.

The new offering will leverage the organization’s clinicians and facilities, along with its 24/7 virtual care, according to an Oct. 9 news release.

The individualized preventive care plans will begin with a digital smart assessment about a member or patient’s current health status, medical history and mental health. A nurse practitioner will review the assessment and any other available data to determine preventive care needs. A plan and recommendations for any additional care will be offered, and patients will also have an option for tele-counseling and coaching.

The new offering is payer-agnostic but optimized for HealthPartners members. According to the news release, the new product could save employers up to 15% on premiums compared to other insurance plans. The new product will launch in January 2024.
