Top 10 Medicaid managed care plans by total enrollment

L.A Care Health Plan is the largest Medicaid managed care organization in the U.S. with nearly 2.4 million enrollees, according to KFF.

KFF used CMS data reflecting enrollment totals as of July 1, 2024 across the 42 states and Washington, D.C. that hold contracts with Medicaid MCOs. Enrollment in PACE programs is excluded from the totals.

The ten largest Medicaid managed care plans by enrollment:

1. L.A. Care Health Plan: 2.4 million enrollees
    California

2. Health Net (Centene): 1.6 million
    California

3. Fidelis Care (Centene): 1.53 million
    New York

4. Inland Empire Health Plan: 1.49 million
    California 

5. Kaiser Permanente: 1.4 million
    California

6. CareSource: 1.2 million
    Ohio

7. Sunshine State Health Plan (Centene): 1.21 million
    Florida

8. Healthfirst: 1.17 million
    New York

9. Vista Health Plan (Independence Health Group): 991,000
    Pennsylvania

10. Superior Health Plan (Centene): 950,000
      Texas
