The cities with the least competitive commercial insurance markets | 2023

Blue Cross Blue Shield of Alabama has more than 90% of the commercial and exchange insurance market share in three cities in the state, the most highly concentrated markets in the U.S., according to the American Medical Association. 

The group published its annual "Competition in Health Insurance" report in December. According to the organization, 73% of metropolitan areas have "highly concentrated" markets, meaning one payer controls more than 30% of the commercial and exchange market. 

Of those, 41 cities' markets are dominated by one insurer with a more than 70% market share.
