The new community access care model is scheduled to launch in Hampton Roads, Va. this summer, according to a May 6 Sentara Healthcare news release.
Services will include embedded neighborhood clinics in affordable housing complexes and mobile health buses to support those struggling with barriers to getting traditional healthcare, such as transportation and time constraints. It will also include partnerships with programs to provide placement for discharged homeless patients from Sentara hospitals.
Goals include building trust with patients who have challenges obtaining care, evaluating local health needs and designing services to fulfill those needs. Other goals include developing programs to manage common diseases and expanding preventive services.
