UnitedHealthcare and Humana are expected to collectively receive 50% of total Medicare Advantage star bonus payments in 2024, while Kaiser Permanente is expected to collect the highest average bonus per enrollee ($516), according to a Sept. 11 analysis from KFF.
Total MA star ratings bonus payments increased by more than 400% between 2015 and 2023, though it will decline by 8% to $11.8 billion in 2024. Total spending on MA bonuses is still higher in 2024 than in every year between 2015 and 2022.
Below are the estimates of how much major insurers will receive. "All other insurers" includes companies with less than 2% of total MA enrollment. "BCBS plans" include Anthem plans (Elevance).
Payers ranked by Medicare Advantage star bonuses in 2024:
- UnitedHealthcare: $3.4 billion
- Humana: $2.5 billion
- All other insurers: $2.1 billion
- BCBS plans: $1.7 billion
- CVS Health: $1.1 billion
- Kaiser Permanente: $976 million
- Centene: $35 million
