Payers ranked by Medicare Advantage special needs plan membership | 2025

UnitedHealth Group has more than 2.8 million enrollees in special needs Medicare Advantage plans in 2025, according to CMS data published Feb. 25.

Medicare Advantage special needs plans cover individuals dually eligible for Medicaid and Medicare, beneficiaries with certain chronic conditions and those living in institutional settings. SNP enrollment grew to 7.3 million in 2025, up from 6.6 million in February 2024. 

Over 1,500 special needs contracts were available in 2025, according to CMS.

  3. Elevance Health: 754,247
  4. CVS Health: 485,459 
  5. Centene: 388,126
