SCAN Health Plan launched a Medicare Advantage HMO with CareMore Health, an integrated care delivery system and primary care provider under Mosaic Health, with coverage beginning Jan. 1.
According to an Oct. 13 news release, CareMore Health Plan by SCAN will be available in California’s Los Angeles and Orange counties. Plan members will have access to 10 CareMore Care Centers, as well as unlimited transportation to the centers. The plan also includes an “expanded network” and a $299 maximum out-of-pocket limit.
“By joining efforts with SCAN, we can bring together CareMore’s personal approach to care with SCAN’s deep experience serving Medicare-eligible patients,” CareMore CEO Sam Wald said.
Mr. Wald previously was a leader at SCAN, while Sachin Jain, MD, CEO of SCAN Group and SCAN Health Plan, led CareMore from 2015 to 2020.
