CMS is imposing a $48,833 fine against a Providence PACE organization in Napa, Calif., according to an April 29 notice from the agency.
The penalty was issued to Collabria Care, the name under which Providence Community Health Napa Valley holds its PACE contract with CMS. The organization integrated with Renton, Wash.-based Providence in 2018 and rebranded in 2023.
The penalty stems from a CMS audit of the organization from Aug. 25 to Sept. 8, 2025, which found the organization failed to follow federal PACE regulations governing service delivery and record maintenance.
First, the organization lacked adequate processes to track and ensure interdisciplinary team-approved services were scheduled and delivered to patients. Specific failures included missed specialist consults, incomplete lab work and missing durable medical equipment.
Second, there was a failure to ensure that service recommendations from employees, contractors, and specialists were reviewed and acted on by interdisciplinary teams within required timeframes, and the organization didn’t document reasons when recommendations were not approved or provided.
Third, the organization failed to arrange or schedule medications and other approved services within required timeframes. Federal rules require PACE organizations to arrange medication dispensing within 24 hours of a physician order and other approved services within seven days.
“CMS conducted a routine audit of Providence Community Health Napa Valley’s PACE program in late 2025 and identified gaps based on a new standard on how certain ordered services are tracked and completed. While the audit did not find broader quality-of-care concerns, we acknowledge the findings and accept CMS’s determination,” a spokesperson for Providence told Becker’s. “Since that time, we have strengthened our tracking and oversight processes to ensure ordered services are delivered consistently and without delay, and we continue to work collaboratively with CMS. Our focus remains on providing timely, coordinated care to the older adults who rely on our PACE program.”
