Under the value-based contract, the four primary care facilities will only accept certain Medicare Advantage plans from Louisville, Ky.-based Humana and none from other payers.
The facilities include Oak Street sites in Indianapolis, Ft. Wayne, Ind., Detroit and Southgate, Mich. The Indianapolis site is the only one currently seeing patients, and the other locations will start seeing patients in coming months.
Oak Street and Humana have a total of six practices designated for the insurer’s Medicare Advantage plans.
