Humana, Aledade ink 10-year agreement for value-based senior primary care

Humana and Aledade are partnering for 10 years on value-based primary care for seniors, which includes a path to full-risk agreements.

Aledade, the nation's largest network of independent primary care facilities, will provide care to Humana's Medicare Advantage members nationwide, according to a March 2 news release.

The two organizations have worked together since 2019 in select markets and shared that their collaboration has led to five percent less inpatient hospitalizations and 12 percent less readmissions. 

By the end of last year, 100,000 Humana members received care under a value-based arrangement with Aledade. The payer also had more than 1,000 value-based relationships across 43 states, including its 235 care delivery facilities.

"Aledade was one of the largest value-based care providers for Humana in 2022, and our relationship will continue our goal to improve access to proactive screenings to identify and treat illnesses early," George Renaudin, Humana’s President of Medicare and Medicaid, said.

As of January, Humana has about 8.6 million Medicare members in all 50 states and 5.6 million Medicare Advantage members. 

Elevance Health partnered with Aledade in August to assist independent primary care physicians with transitioning to value-based care.
