An in-home care program helped Independent Health reduce preventable hospital admissions, readmissions and emergency room visits, The Buffalo News reported Jan. 11.
The Buffalo, N.Y.-based payer launched the Care for You program in late 2021. The program has 2,200 Medicare Advantage members with complex, chronic conditions.
The Care for You program reduced preventable hospital admissions by 54 percent and readmissions by 45 percent, The Buffalo News reported. It also reduced emergency room visits by 23 percent and inpatient hospitalizations by 33 percent.
The company projects more than 4,000 Medicare Advantage members will join the program by the end of the year and rolled out Care for You to Medicaid members in December.
