Specifically, 2.2 percent of individual health plan policyholders filed 50 percent of claims last year, state Commerce Department officials said Nov. 14. As a result, premiums rose for healthy customers as well.

The Minnesota Department of Commerce is advising any solution to curb rising premium costs address the unbalanced risk pool, according to the report.
