Elevance Health has partnered with the National Association of Community Health Centers to integrate food as medicine initiatives into primary care services for Medicaid members at Community Health Centers.
Elevance will train CHC primary care teams to identify and support patients at risk for diet-related chronic diseases. The program will offer personalized nutrition interventions, such as medically tailored meals, nutrition coaching, and referrals, according to an Aug. 18 news release. The initiative will also leverage telehealth to help improve chronic disease management, patient engagement, and support preventive care.
CHCs provide care at more than 17,000 locations across the country. The partnership with Elevance aims to expand the new services and scale them to reach more Medicaid patients, ultimately impacting the 34 million individuals served by CHCs nationwide.
Elevance’s broader food-as-medicine strategy includes over 60 programs across its health plans, which focus on chronic conditions like diabetes and cardiovascular disease.
