More than 14,000 of Cook County’s patients reach Medicare eligibility each year. The MoreCare Medicare Advantage plan will offer patients four products, including a traditional Medicare Advantage drug plan and a chronic condition special needs plan for residents who are HIV-positive. This will be Cook County’s first HIV-specific chronic condition special needs plan, the organization said.

The MoreCare Health Plan will begin enrolling members Oct. 15 for coverage effective Jan. 1, 2020.

MoreCare is a subsidiary of Chicago-based Medical Home Network, which is a nonprofit community-based care model serving Cook County’s safety-net patients.
