The Humana Cleveland Clinic Preferred Medicare Plans are effective Jan. 1. The joint plans — a Medicare Advantage health maintenance organization and a Medicare Advantage dual-eligible special needs plan — sport $0 premiums and copays.

The plans build on a value-based agreement between Cleveland Clinic and Humana. Members selecting the plans will gain in-network access to the clinic’s hospitals, physicians and outpatient centers.
