The payer will use Socially Determined’s “SocialScape” platform to identify people who face elevated health risks from financial strain, food insecurity and health literacy challenges, according to a May 24 CareFirst news release.
The platform will give the payer the insights necessary to build highly tailored responses to people’s specific needs, according to the news release.
CareFirst said in the news release it will use the platform to help employers better understand factors beyond clinical care that drive poor outcomes for their workforce. It will also be used to optimize philanthropic investments in new community-based programs, partnerships and interventions and ensure care management teams are effectively reaching and supporting people with complex socio-clinical needs.
