CalOptima invests record $50M in value-based care for California health centers

CalOptima is investing a record $50 million over the next five years into 26 community health centers in Orange County, Calif.

The Medi-Cal plan awarded the grant to the Coalition of Orange County Community Health Centers to improve care access, outcomes and the safety-net system, according to an April 13 release provided to Becker's.

The coalition will design a health center-specific implementation plan for measurable improvements in patient experience and health outcomes that will support:

  • Establishing medical homes for select populations, such as those experiencing homelessness
  • Improving care access and coordination, including care that will support individuals returning to the workforce
  • Reducing avoidable hospitalizations and readmissions
  • Participating in a health information exchange and data interoperability

"As the backbone of the county's safety net, community health centers provide essential care to underserved populations that often have the most complex needs and least resources," CalOptima CEO Michael Hunn said. "This critical funding will support the goal of providing value-based care for more than a quarter-million CalOptima members and others who are among the most vulnerable."
