California health plan uses combined Medicare, Medicaid funds for community program

The Health Plan of San Mateo (Calif.) is one of six county health plans in California that manages both Medicare and Medi-Cal — the state’s Medicaid program — and it’s using the funding that comes with that responsibility to broaden what health insurance means.

Specifically, the health plan used the money to kick-start its Community Care Settings Program, which provides patients in nursing homes with resources to transition back into the community, according to KALW

The program was created as a response to nursing homes that were overcrowded by patients who were occupying the space for longer than was needed because they had lost their homes during their initial stays. 

Community Care Settings can provide security deposits for housing and then connect beneficiaries with agents who can help with Section 8 vouchers. The program also covers case managers who work with patients transitioning out of nursing homes. 

The initiative is both investing in people and the health plan itself, with an estimated 30-35 percent cost savings if patients can be transitioned back into communities.
