BCBS of New Mexico pens value-based, at-home care agreement with Addus HomeCare

Blue Cross and Blue Shield of New Mexico entered a value-based agreement with Addus HomeCare subsidiary Ambercare, which will implement Addus’ services statewide.

The agreement, dubbed “Stay Healthy at Home,” will allow Blue Cross and Blue Shield of New Mexico and Addus care coordinators to “remove barriers to service” and “mobilize community-based clinical care” to avoid hospital visits when applicable, according to the Nov. 16 announcement.

Addus will also train home aides to identify changes in a member’s condition so they can coordinate next steps with their primary care physician. 

“We are pleased to enter this agreement with BCBSNM, to help achieve better health and financial results for BCBSNM as well as improved member satisfaction and outcomes,” said Dirk Allison, chair and CEO of Addus. “As a provider of comprehensive home-based care, we have a significant opportunity to work with the BCBSNM team to promote better outcomes. The ‘Stay Healthy at Home’ program will benefit both our dedicated team of caregivers and the clients we serve across New Mexico.”
