The purchase is SCAN’s first acquisition of an external company. The Long Beach, Calif.-based organization will merge TRG with its existing in-home care program, HealthCHEC, to form Homebase Medical. The new in-home care program will provide palliative care, chronic disease management, care transition management and personal health assessments for SCAN Health Plan members and other health plan policyholders, according to a May 26 press release.
“We are excited about adding TRG to our portfolio and view our first-ever acquisition as an investment in the future of SCAN,” President and CEO Sachin Jain, MD, said. “Not only will it help further SCAN’s mission to keep seniors healthy and independent, but it will also bring greater efficiencies to our work as we expand our in-home clinical delivery capabilities.”
