The Cigna Group’s total customer relationships declined 6% from Dec. 31, 2025, down to 177.6 million.
Cigna reported its third-quarter earnings Nov. 5, attributing the decline in relationships to pharmacy and behavioral customers. Pharmacy and behavioral customers decreased 5% and 19%, respectively, from the end of 2025. Cigna said it experienced client transitions, lower membership from health plan clients and the termination of a government contract. However, total medical customers increased 2% from the end of 2025, reaching 18.5 million.
The company still boosted its adjusted earnings-per-share outlook from at least $30.45 to at least $30.60.
Cigna had $1.4 billion in profit for the quarter, down from $1.9 billion during third-quarter 2025. The company said this was due to the absence of an after-tax gain on sale, which was reflected in the earlier earnings. Adjusted income from operations was flat, holding at roughly $2.1 billion for the quarter.
Cigna’s Evernorth Health Services reported adjusted revenue of $63.8 billion, up 6% year over year. The division’s pre-tax adjusted income from operations was $1.8 billion for the third quarter, down 4% from the same period in 2025.
Cigna Healthcare, the insurance division, had $11.7 billion in adjusted revenue, up 9% year over year. Its pre-tax adjusted income from operations was $1.2 billion, a 15% bump from third-quarter 2025. The medical loss ratio was 85% for the quarter.
