Independence Blue Cross is bringing Bala Cynwyd, Pa.-based Patina Medical Group into its provider network, the payer announced Feb. 22.
The primary care provider focuses on patients 65 years and older and is available to two of the payer's Medicare Advantage plans in 2022.
Patina's model uses a team to provide tailored care to patients through virtual and in-home visits. The team includes a dedicated primary care practitioner and a "care champion" that serves as a touch-point for any inquiries.
"The health care system can be complicated and frustrating for Medicare Advantage members to navigate," said Heidi Syropoulos, MD, medical director of government markets at Independence Blue Cross. "That's especially true for those who are managing multiple health conditions or who have challenges accessing office-based care. Patina helps simplify things for members by offering high-quality primary care that is easy for them to access in their homes and through the computer and phone."
