UPMC Health Plan, InterWell Health partner on kidney care

UPMC Health Plan and value-based kidney care provider InterWell Health are partnering on care coordination for members with kidney disease, InterWell said Oct. 12.

According to a news release, proper care coordination, lifestyle support and education can delay the need for dialysis treatment for patients with kidney disease by several years. 

The initiative with InterWell will cover members across multiple UPMC Health plans, including Medicare Advantage and commercial plans. 

“We support these members with real-time access to multidisciplinary care teams and peer mentors, all in close partnership and coordination with our network of high performing nephrologists,” InterWell Health Chief Medical Officer George Hart, MD, said in the news release.
