Tech-enabled healthcare company Included Health is launching health plans for self-insured employers Jan. 1, 2028.

According to an Oct. 1 news release, Included Health will offer two plans that aim for total cost of care savings from 6% to 15%. Employers can select either a comprehensive or flexible configuration, the latter working with current carrier networks and third-party administrators. Both plans will also offer dynamic copay-focused benefit designs and TPA integrations and capabilities, the release said.

“Included Health Plans give employers a clear value proposition and flexibility: Supercharge your existing network, or build a supercharged network to make the highest-quality care the easiest choice,” Owen Tripp, Included Health’s co-founder and CEO, said.

The announcement follows Included Health’s acquisition of Firefly Health, a clinically integrated health plan and advanced primary care company. Firefly’s former CEO, Fay Rotenberg, is now serving as the health plan president for Included Health.

“When we founded Firefly, our thesis was to invert that broken equation and build half-priced healthcare that’s twice as good. Firefly built the full-stack model to realize that vision, bringing care delivery, network, navigation and coverage together into one integrated plan experience,” Ms. Rotenberg said. “Now that Firefly is embedded within Included Health, we can scale that model and its outcomes more quickly and give employers a sustainable path to a lower total cost of care while actually elevating the member experience.”

The plans’ in-house, nationwide network will consist of more than 1,000 virtual care providers. Members will also have access to in-person care through more than 2,000 Firefly Nearby partners. Specialty care will be available through Carrum Health, Oshi Health, Pomelo Care and Hinge Health, the release said.

The plans will be available to self-insured employers with 5,000 or more employees.

At the Becker's 5th Annual Fall Payer Issues Roundtable, taking place November 2–3 in Chicago, payer executives and healthcare leaders will come together to discuss value-based care, regulatory changes, cost management strategies and innovations shaping the future of payer-provider collaboration.
