The partnership involves Humana Medicare Advantage members in FullWell’s Colorado Health Neighborhoods network. The agreement expands Humana Medicare Advantage members’ access to more than 150 FullWell primary care physicians in Denver. In addition, participating practices will use FullWell and Humana population health management tools to identify gaps in care, manage medications and identify frequent emergency department users, among other strategies to improve members’ health.
