Effective April 1, Aetna members with individual Medicare Advantage coverage gained access to Oak Street Health’s primary care centers in Indiana and Michigan at in-network rates. The agreement expands Oak Street Health’s network to 1.6 million Medicare-eligible Aetna members.
Oak Street facilities offer 24/7 access to a physician and care coordination teams. The primary care chain said its value-based care strategy has reduced Medicare hospitalizations in other Oak Street facilities by more than 40 percent.
