Cook County Health, a safety-net hospital in Chicago, partnered with Medicare Advantage plan MoreCare to offer benefits to eligible residents in Cook County, the health system said Oct. 10.
More than 14,000 of Cook County's patients reach Medicare eligibility each year. The MoreCare Medicare Advantage plan will offer patients four products, including a traditional Medicare Advantage drug plan and a chronic condition special needs plan for residents who are HIV-positive. This will be Cook County's first HIV-specific chronic condition special needs plan, the organization said.
The MoreCare Health Plan will begin enrolling members Oct. 15 for coverage effective Jan. 1, 2020.
MoreCare is a subsidiary of Chicago-based Medical Home Network, which is a nonprofit community-based care model serving Cook County's safety-net patients.
