In a July 31 news release, the agency unveiled the Guiding an Improved Dementia Experience, or GUIDE, model.
The model will provide people with dementia and unpaid caregivers with care coordination and support services.
The model will provide one-time infrastructure payments to community support providers, per-beneficiary per-month payments to organizations providing care coordination, and respite care payments, according to a fact sheet.
In the news release, CMS Administrator Chiquita Brooks-LaSure said the agency has made “tremendous progress” in improving care for people with dementia, but the new model will address additional gaps in support.
“People living with dementia and their caregivers too often struggle to manage their health care and connect with key supports that can allow them to remain in their homes and communities. Fragmented care contributes to the mental and physical health strain of caring for someone with dementia, as well as the substantial financial burden,” Ms. Brooks-LaSure said.
The model will run for eight years, beginning July 1, 2024. Organizations can submit letters of intent to participate in the program through Sept. 15.
